First Visit

During your initial visit to our office, we will conduct several examinations to become acquainted with your current dental health and evaluate possible treatments. If the diagnosis is a root canal issue, we would immediately schedule you for a treatment appointment. In situations of extreme pain, we would handle your treatment as an emergency and do our best to ease your pain the same day. Once we have stabilized your pain, we will schedule you for a follow-up visit.

We will need the following information during your initial visit examination.

  • A list of all your current medications
  • Your reference form
  • Any X-ray records you may have

We will also need your insurance card if available. If your insurance card is not available, you can provide the phone number, group name, and subscriber number. These details will help with the quick processing of your insurance claim.

IMPORTANT NOTE: It is mandatory that all minor patients (those below age 18), come with a parent/guardian for an initial visit.

If there are ailments you are taking medications for, it is important to let us know. These ailments and medications may include diabetes, high blood pressure, rheumatic fever, heart medications, aspirin, etc.

X-Rays

Please forward any X-ray results of examinations conducted by your doctor/dentist, as they may help with a diagnosis and possible treatment plan. If digital radiography was used, you can forward the results by email to our email address. We will produce extra copies in our office as necessary.
